However, it is only 1 sheet of paper, so no major problem to carry alongside ones passport "in case", eh?
Exactly. In theory it's not needed but it's a couple of sheets of paper why would you not take it just in case.
I've never been asked to provide details of my ESTA - both the airlines and immigration can access it electronically but I do take a copy of the approval with the ESTA reference number just in case. It's only a single sheet of paper you can get rid off once past US Immigration.

If you go the ESTA website there is the facility to retrieve an application - all you need is certain personal information.
