The drive is between an hour and half and two hours, depending on the traffic and the taxi 120 - 130 USD one way.

No shuttles, only a taxi
Well you could take a bus, but it is a bit complicated, you cannot get on a bus from the airport, still would have to take taxi to La Romana city and at the end you won't save much but will save lots of time :)
Not easy, if you can, get a bus to punta cana airport and take a cab from there, if not get off either at the gas station in veron or plaza san juan (always taxis there) in Bavaro and take a cab from there, should cost $30 taxi fare.
