It only lasts 15 - 20 minutes. I don't know how early you should get there, but it is usually pretty crowded in St Peter's Square, even more so on Sundays during the Pope's appearance, and the lines to get through the metal detector gates are long. But there are 2 things I found very useful when planning our visit there in April:

1. Check the Vatican website to be sure the Pope will be there that Sunday: vatican.va/various/…udienze_en.html

2. You can hear him very well, but if you want to see him, you'll need to be in direct visual line of the tiny window where he appears, so make sure you know which window it is and get there well ahead of the Angelus, because once it starts, it'll be impossible for you to move there.
