Public areas in hotels (restaurants, bars, lobbies etc) are required by law to be non-smoking. Hotels are allowed to permit smoking in designated bedrooms, although many choose not to and prefer to remain non-smoking throughout.

If you want to be guaranteed a non-smoking room, best check that the hotel has a strict non-smoking policy, or it's possible that you'll be occupying a room previously used by a 60-a-day chain smoker and will stink.

Not likely, as most hotels are non-smoking, but possible.
