What about Tortuguero? There are several nice lodges there with pools. Most of them feature a "package" deal that includes transportation from San Jose - but - you can contact them to see what you can work out about transportation from Puerto Viejo.

Another option would be somewhere in the Turrialba/Orosi area.

How about spending a few days in the Arenal area?

Definitively not Bocas del Toro, after a month in Puerto Viejo. Tortuguero is a wise advice, being so different then PV (and the rest of Costa Rica).

Personally, I would look at a place to "cool down" after a month of Caribbean sun. I would head into the mountains; areas like Turrialba, Cerro de la Muerte, places near Orosi Valley, or even Monteverde. You will be able to find many small lodges (but no AI!!), some will even have a pool (after a month of Caribbean beach?!).

All of these places are reachable by local bus, however none as an "easy reach" (apart of Cartago and Tortuguero).
