Centers for Disease Control and Prevention health information for travelers to Brazil:


Brazil does not require a certificate of vaccination to enter the country.

You should speak with your doctor about shots for traveling abroad.

IF you were to get one from the US, it would be a yellow fever.

We did not get it and we were okay, our friends traveled a month after us and did get the shot.

Personal preference. If you wear long sleeves and pants mostly, nothing to worry.

From the usual touristic places, its very advisible to take vaccine if you are going to the Amazon Forest. If you stay in states that are in shore or close to, there is almost no risk of yellow fever.
