When we stayed in Manuel Antonio earlier this year we did
-a tour of Manual Antonio National park
-we went horseback riding
and lounged by the pool and beach.
Lots of restaurants to chose from. You'll have a great time.
In Monteverde check out the agricultural tour at the Heliconia Ranch - its horseback riding and a farm tour, great for kids. Selvatura is also great for the zipline, and the repitle and insect exhibits they have onsite. They will tandem zip line with the kids if you ask (where the guides to all the braking, etc).
The Jungle Crocodile Safari on the Tarcoles is a nice trip to do on the drive down to Manuel Antonio - break up the drive with the kids and I'm sure they would love seeing the crocs. Si Como No has a great night hike through their reserve to see frogs and spot them with flashlights. Damas Island by boat or kayak is also really nice for wildlife spotting.
