$35-45 depending upon where you're going and traffic.

It cost us $40 plus tip going from airport to Handlery Hotel at Union Square. By the way, great hotel/staff/location.

There are no tolls between the airport and downtown.

However, if you have a cab cross a bridge, yes, you would pay the toll. (In fact, if you had an SF cab take you to Sausalito -- that's the no toll direction -- I'm pretty sure you'd have to pay the cab's toll on his return trip.)
