What kind of tours are you interested in? Whole day bus tours? Trolley Tours? Walking Tours? And what kinds of sites interest you? Architecture? Culture? History? Your questions are much too general.

You should definitely do the freedom trail. You can either hire a tour or just do it on your own. Our "little Italy" is called the "North End" and you can definitely do that on your own. I'd say typical September weather is in the 60s.

Mid-September is generally in the 60's and 70's, though cooler at night. The Fenway Park tour is definitely worth a go. Duck Tours are very popular, as are the Old Towne Trolleys. You can walk the Freedom Trail, either on your own or on a guided tour. A lot of people like to visit Harvard in Cambridge.
