Yes sand flies are an issue.....you are wise to be prepared!
We went to all three Sandals when we were there, and I didn't get bit by any sand fleas. Not saying they're not there, but I didn't have any issues.
If you spend a bit of time outdoors you'll likely encounter them because they are definitely there! Most of the commercial repellants don't really work very well and many contain toxic ingredients for your skin.
Here's a good natural formula for a cream that will really help to keep them away:
50g net zinc cream
6 or 7 drops of citronella oil
2 drops of vinegar (apple cider vinegar works great here)
The zinc cream helps the application last longer. The citronella is the active ingredient derived from the lemongrass plant.
You can also make an oil to rub on using baby oil and citronella oil.
Beware of seaweed churning in the shallows - I'm not sure what bit me, but when I was in the water at Morgan Bay some seaweed that had come in with the tide got wrapped around my legs. I felt this strange kind of scratching sensation on my legs that freaked me out, so I got out of the water. A couple of hours later my legs were dotted with bites that were excruciatingly itchy. I didn't know what they were so I left them alone, which proved to be a smart move on my part as scratching can lead to infection. To make a long story short, some Lanacaine and Benedryl cleared them up.

Yes, there are sand flies, usually found on beaches that have not been sprayed with pestides.
The local solution is home-made Coconut Oil applied to exposed skin.

Thank you for all the responses. We first encountered them on a Cuban beach two years ago and we were not prepared for them. Now that we know they are there, we know what to expect.
