After several years traveling and living in the DR, this is by far my favorite B&B, and Cayo Arenas (not Cayo Levantado, as a previous traveller stated) is one of the only coral reefs still ALIVE in the island..

Punta Rucia is a small virgin Beach village (about 500 population) about 75Km down the road West off Puerto Plata... a 2 to 3 Hr. Trip from Santiago STI or Puerto Plata POP airports.
It's also the sail off point to a small sand "island" about 20 boat minutes away called "Paradise Island" or Cayo Levantado". That sand dune really, is only a few 100 m2 large and is completely surrounded by a protective ring of corrals. Tourists are brought there daily by bus and boat to snorkel alongside a myriad of fishes just happy to get some bread and pose to those bringing underwater cameras along.
We live near Puerto Plata and have come to go on that little "island" a year ago. We explored the surroundings on the main land too and found that there isn't much around really, but that this is maybe an attractive. There are 3 wonderful and very flat beaches along two bays with Christal clear water which are mostly visited by "locals" coming down from the cities like Santiago during the weekends. IF you want to hang around Dominicans in "their" environment, then the weekends are for you, if you prefer to enjoy pristine virgin beaches virtually alone, Mon-Fri is for you. There is a strip seafood and fish restaurants along the most Eastern beach... they can be good, at times even excellent, but one has to know the prices because since business is not so great, some try to be somewhat over priced. We had a great and deliciously prepared lobster for the equivalent of USD11.oo just last weekend... so, that's OK.

Now to Casa Libre.
Casa Libre is an independently owned, and run... really SELF BUILT little and SIMPLE "Island Outcast" Paradise owned by a lovely but unlikely couple of a Geman lady and an French gentleman who've been together and don't speak each other's language! The cabins are "Dominican Style" wooden structures painted in happy colors like one will observe out in the Caribbean countryside. It's SIMPLE, yet very well maintained, and as clean as nature allows.
It all sits on a private hill at a point in between both bays which leads all the way down to the pristine beaches. The view is breath taking from every one of the 3 cabins and their cana roof covered wooden balconies. Each room is equipped with a queen sized bed covered with a mosquito net "tent" a chair, a wall fan an that's about it.
I was asked to also stress that they produce their electricity with solar panels and the help of a small generator for the few cloudy days and thus obviously try to keep electric consumption as low as possible. Hair dryers and game consoles are not welcome. So, there's is no hot water, no TV's, NO air conditioners (Sea breeze and wall fans), NO INTERNET, WiFi or other entertainment besides a few dogs and cats and inoffensive lizards having fun with each other or... NATURE. The owners LOVE and enjoy every minute of their life or freedom and will do everything to help their guests do the same, but refuse to cater to the "luxury" crowd as that's not the way they see and live their life and also just not are setup to. You however don't have to be a Hippie to enjoy this. It's gorgeous and even my very luxury addicted niece just opened up and forgot about Facebook and all the tech-BS after two days.
The owners will share their wonderful breakfast with exotic fruits and fresh juices with their guest, but are not setup as a restaurant, yet are open to cook on pre-order within the limits of what the local market has available.

We loved Casa Libre and will come back with our guest.

This is a place which is well worth a visit. It is a bit tricky to get to but is well worth it. A quirky little guest house with very individual room designs. Our room was at the top of the hill and had amazing views out to Punta Rucia and the ocean. The hotel does not have the mod cons of others but this is what gives it its charm. No wifi, no air con (there are fans), peace and quiet and simple home cooked meals. We were able to go sea kayaking in the mangrove swamps
