Stayed 2 nights in May 2013. It was a bit of a nightmare to find as Baden-Baden is a maze of pedestrianised streets that our GPS didn't recognise. Parking for the hotel was about a 5 minute walk away in a large public car park (20Euro for overnight parking). The hotel is very central with all the main town attractions within easy walking distance.
The entrance to the hotel is, strangely, through a cafe. You go in through the main cafe doors and one of the waitresses checks you in then takes you up to the room. I felt a bit uncomfortable walking back and forth through a busy cafe laden with luggage.
The room we stayed in was above the main square with a tiny balcony (no space for a chair or table). The room was very clean but the bathroom had a very slight urine smell about it, and the bathroom is absolutely tiny (a 'larger built' person almost certainly wouldn't be able to use the shower).
Noise in the busy square below starts at about 6am with beer barrel deliveries, rubbish collection, flower display deliveries, street cleaning etc and continues all morning. Noise from the restaurants below finished at about 10pm but it was very cold and rainy when we stayed and I suspect the noise carries on later in good weather. Even though it was cold outside, the room was very warm, but the street noise meant we couldn't sleep with the windows open. There was fan supplied in the room.
Nice breakfast (strangely served in the cafe downstairs), good choice and pleasant staff. The owners of the hotel seem to own several establishments on the square so people pour in from those places too for breakfast.
I would probably stay again if I ever went back to Baden-Baden.
